Dear Visiting Angels Applicant:

Enclosed is a confidential questionnaire including assessment questions. In order to be considered for a position, it is necessary to complete all blanks on the questionnaire and the assessment questions. The professional references must all be completed, including dates of employment and phone numbers. This information is essential in our being able to process your application. Once the questionnaire and assessment questions are completed, please send it to the address shown on the questionnaire , or you may fax us at 832-632-2256. Both the questionnaire and assessment questions must be received within one week after date of the original mailing to be considered. After receipt of the questionnaire personal references, professional references, criminal history, Employee Misconduct Registry and Nurses’ Aid Registry checks will be conducted. Once the checks have been completed your questionnaire will be reviewed. If requested for an interview you are expected to come dressed in scrubs with closed toed shoes or tennis shoes. Please keep in mind the interview process is a three step process with our agency. The first step being the telephone pre-screen, second step interview and the last step orientation. In order to work for our agency and be added to our registry for work all steps of the interview must be completed, no exception. You will be evaluated based on your ability to complete the questionnaire process according to the agency policies and procedures. Expectations include “showing up on time” for the interview appointment that you have accepted and agreed to. If you are late, cancel or fail to show up, you will not be considered for employment. This also applies for orientation and the meet and greet with clients. During your interview, orientation and meet and greets with clients, your cell phone should be turned off. Do not bring friends or family to these meetings as it is unprofessional. With Visiting Angels, it will be entirely your choice as to which cases you decide to accept or reject.

To complete your questionnaire, please include a copy of a valid driver’s license, social security card and current automobile insurance card. The copies of these documents are to be returned with the questionnaire and assessment questions in order to be added to our caregiver registry for up and coming positions, as well as to gain an interview. In addition, a current Tuberculosis test or test results are required. Copies of the certificates or cards you may have received from additional training such as your Certified Nurse’s Aid Certificate, CPR or HHA. Please understand these certificates are required in order to qualify for a job. The State of Texas requires that these certificates be available if you have received them.

It is necessary to satisfy the licensure requirement for Personal Assistance Services, which are the services we provide to our clients. In order to do so we must make sure all the paperwork required is provided. If you do not have current documentation, it will become necessary to bring it current before we are able to offer you a position as a caregiver with our agency.
